First, sorry this is so long, I was unable to send it as attachments so I pasted everything into this e-mail.  

I did not receive any copies of formal 504 plans.  I am including a draft of the 504 plan accommodations that I am requesting for my daughter (see bottom).  Please understand that has not been put into final form and may not be formatted correctly for a 504 plan.  If anyone does have a formal plan I would still love to compare plans.  You can go to www.allergysupport.org and look under the heading "Going to School".  They have a sample plan and some articles about 504 plans. 


Many people asked me what a 504 plan so I am also including that information (this is from allergysupport.com):
WHAT IS SECTION 504? Section 504 is the abbreviation for Title 34 Section 504 of the Rehabilitation Act of 1973. It is a civil rights law that prohibits discrimination on the basis of disability.  It applies to all institutions, including public schools, which receive financial assistance from the federal government.  In the public school context, children with disabilities may be protected under Section 504, IDEA or both. Accommodations for 504 plan



  1.. Emily will be placed with a teacher knowledgeable about Celiac Disease (CD), if one is available for her grade level.  If there is not a teacher who is knowledgeable, the teacher she is placed with will be trained to work with CD prior to the start of the school year.  
  2.. All teachers and other staff who will be responsible for Emily during a part of the day will be trained to work with CD.  This includes the art teacher, music teacher, Spanish teacher, librarian, gym teacher, computer teacher, supervising teachers for recess, supervising teachers for children who stay in for recess, cafeteria staff, and bus driver.  In addition, all substitute teachers in any of these positions will be trained to work with CD.
  3.. In the event that a substitute teacher is required for the main teacher and the main teacher is not able to communicate with the substitute verbally prior to the absence (i.e., sudden illness) then the parents will be notified and given the option to provide instruction to the substitute.  A written summary of CD information will also be provided to the substitute.
  4.. In the cafeteria, Emily will be monitored by a staff member trained in CD at all times.  The staff member will insure that no food is given to Emily other than what is packed in her lunch.  Emily will bring a paper placemat in her lunch bag and the staff monitor should insure that she uses the placemat for her food.  The staff member will insure that no cross-contamination occurs due to crumbs, shared condiments, or shared utensils.  Students seated next to Emily will be monitored to insure that they do not touch her lunch or her during lunch and that they keep their own food, food crumbs, and utensils away from Emily.  If a child seated next to Emily cannot follow these procedures they will be moved away from her.  
  5.. Emily will have unrestricted access to the toilet.  
  6.. All lunches will be provided by the parents and sent to school daily.  All snacks (to be kept in a designated gluten-free drawer) will be provided by the parents monthly or as needed.   Special gluten-free treats (frozen cupcakes, GF chocolates, etc.) will also be stored at school for those "surprise" occasions when a treat is presented to the rest of the class.  On those occasions that a special treat is planned (i.e., pizza party, birthday cupcakes, etc.) the parents will be notified and will provide a GF treat.  
  7.. Non-food items that Emily uses must also be GF.  These include art supplies (paints, play doh, glue, markers, etc.) and personal care items (tissues, soaps, lotions, etc).  Parents will provide GF arts and craft supplies and personal care items for Emily.  As special craft projects come up during the year, parents should be notified to bring in needed GF supplies.  Ink pads or stamps usually contain gluten and Emily must not have stamps put on her hands.  Stickers are questionable and the parents request that these not be put directly on her person.  Envelopes with adhesives that you lick may contain gluten and Emily must have a GF alternative such as a peel and stick label.
            9.   For all special parties (Christmas, Halloween, etc.) the parents will be given             the opportunity to be present to monitor treats.  If the parents are not available, a       staff member trained in working with CD will be available to Emily.                                                                                                                                        

            10.  For all school field trips the parents will be given the opportunity to be        present to monitor food.  If the parents are not available, a staff member trained in         working with CD will be available to Emily.

11.    Emily will be excused from all "jobs" or activities in her classroom that exposed her to gluten.  For example, she should not pass out snack, clean up food trash, or wipe classroom tables after snacks.  

12.    All students (and staff members) in Emily's class will wash their hands immediately following lunch, all snacks, and all exposures to other gluten containing substances (i.e., play doh, paints, etc.).   All tables will be wiped after these activities by a staff member.

13.    Emily will have a "card" with her picture, name, and basic information about CD on it.  Copies of this card will be made available to all staff working with her.

14.    All incidents of possible gluten exposure will be reported to the parents immediately.  All symptoms of possible gluten exposure (diarrhea, constipation, straining on the toilet, crying on toilet, complaining of tummy aches, using the toilet frequently) will be reported the parents immediately.  

15.    If teachers or staff members are unsure of whether a food or non-food product is gluten free:

1.      The product should not be given to Emily if the GF status is questionable.

2.      Contact Mom for information on GF status.  This may be done at any time during the day via cell phone or home phone.

3.      If Mom does not have GF status information she will research and inform teacher of GF status.  If item is not GF, Mom will provide GF alternative, if possible.
