Usually, a therapist will fill out the reccommendations and the doctor will
sign them.  He may also need a script before getting the driving eval done.

most states require a medical opinion froma doctor
before they would allow him to get a learners permit.
the doctor would also have to say if he needed special
adaptations, ie: hand controls.
