Does anyone have any information about the ketogenic diet?

1. Does it require a doctor's prescription?

2. Is it an FDA approved thing?

3. Does insurance reimburse?

4. Is it considered "medically necessary"?

5. Does it have any side effects? Harmful side effects? If so, what? Coma?
Death?
