Reminds me of an experience I had with a Millwaukee Hole Hawg 25 years ago
(when I was young and even more foolish).  I was drilling out a maybe 3/4"
hole in a 3/8" thick steel flange on a hydro turbine, standing six feet in
the air on the narrow 4' diameter flange.  When the drill bit on that last
little bit I was not ready and the thing took off, finally ripping the cord
out of the drill before it stopped.  I don't know if I had the trigger
locked in or whether it was just momentum that kept it going.  

I was the manufacturer's rep, fresh out of college, borrowing tools from the
customer to make a repair to our equipment.  Ah, the good old days of my
youth.  Underpaid and overworked and on a seriously steep learning curve.

My last impression of him I was using an auger drill to drill through a 
floor joist, working overhead from a step ladder. You have to be fairly 
well braced and determined to drill a hole in order to do this.
