JOB ANNOUNCEMENT

Community Partnerships has an exciting opportunity for a fluent Spanish speaking professional to join our Case Management team.  In partnership with Dane County, Community Partnerships is responsible for providing case coordination services for children and adolescents with severe emotional disturbance and their families.

Job Title:                       Case Coordinator--fluent Spanish speaking

Qualifications: -Bachelors degree 
                -Relevant experience working with children, youth and their families
                -Demonstrated writing ability
                -Demonstrated ability to work as a member of a team
                -Basic computer competency
                -Ability to travel within the state

Preferences:    -Experience working with people of diverse cultural backgrounds 
                -Bilingual
                -Knowledge of the Dane County Juvenile Court system
                -Knowledge of Dane County child and juvenile resources
                -Good public relation skills

Job Duties:     -Assemble and coordinate family centered, culturally competent treatment treatment teams consisting of professionals and natural supports
                -Facilitate team meetings
                -Develop treatment plans that are culturally competent, out come oriented, and community based
                -Identify and collaborate with community resources to build on client strengths
                -Document outcomes of treatment plan
                -Evaluate family progress in reaching identified goals
                -Engage family in treatment process
                -Appear in court as needed      	

Work Hours:                     Coordinators work a flexible schedule, approximately 40 hours per week.
                                Evening hours may be required.

Salary:                 $30,000 with benefits

Transportation:         Valid driver's license, car insurance and reliable transportation are required.
