I finally gave up on this and called the vendor...eventually they gave up
on it and gave me a RMA.
Armed with the RMA I decided to try one last trick......I shorted the BIOS
for 30 seconds to clear it and then loaded BIOS defaults. The floppy drive
immediately started acting normally and all problems disappeared. I then
made the BIOS changes one at a time to see if any particular one was
causing a problem. Everything appears to be working perfectly now.
