The 504 must be filled out by the physician as well.
And yes, they need to validate your claim.
I told mine what to write, besides the prescribing
info, of course.
The school had Ben eating in the same place everyday
until the 504 was in place, which was very isolating
for him.
I emphasized this social aspect in my letter to the
district requesting a lunchtime para dedicated to Ben.
