Mike,

I would make a boot disk from a different machine with the same version
of Windows.

Boot from the floppy.   Then FDISK the drive, remove and reinstall the
partition to remove all question that a virus is involved, (be sure the floppy
is locked to "read-only" to make sure it is not infected if a virus is present).

Now, there should be no reason why the system will not format from the
floppy using the 'FORMAT C: /s'  command.
