Looking at the Microsoft support site gave me two ideas:

1. This can happen if the CMOS is set for the wrong kind of floppy disk.
That is, if the floppy is a 3.5" and you have it set as a 5.25" in CMOS.  I
doubt if this is your problem though - just something to check.

2. Make sure you have FORMAT and SYS on your floppy.  Run the FORMAT
without the "/s" extension.  Once the format is done (assuming it now
works), run SYS C: to transfer the system files to the hard drive.
