Frank,

Have you upgraded to Windows 95 or 98 recently?  My mouse behaved similarly
in DOS mode following an OS upgrade.  A new generic serial mouse driver (for
DOS) solved the problem.

Does this happen in Windows or only in DOS mode?
If in Windows, the Windows Mouse Driver is probably corrupt, delete it and
replace it from your Windows CD.  If in DOS, the driver is missing, i.e. not
loaded in CONFIG.SYS or AUTOEXEC.BAT.  Load the appropriate driver in you
start up file(s).

Is it a serial mouse?
Many serial mouse drivers are interchangeable (i.e. not manufacturer
specific).
I'm using a Microsoft Serial Mouse, and a Logitech Serial Mouse Driver (less
buggy than the MS one).
Find the most current driver you can and give it a shot.
