I think I have a copy, but my material is on loan.  My mother lives in
another state, and she has it.  When I get it back I'll let you know, and
see if you still need it.  If you can get to a decent library you should
be able to burn a copy yourself.

However, I think the on-line Eaton paper mentioned recently on this list
encompasses the relevant info, and is more updated.  Is this correct,
anyone?
