Viki,
It sounds like you are looking for a tool for conducting assistive
technology assessments. One place to start is my package of articles on
how to choose assistive technology. You can find this at

Also the tech act project in Wisconsin has developed a complete evaluation
and assessment package on at. I don't have that project's number, but
your tech act project in Tennessee should be able to provide it for you.
If they are reluctant to have over the materials, put up a fuss. the
materials were developed with federal funds and should b shared
nationally, particularly if one is willing to pay any nominal fees for
duplication and mailing.
Finally, the University of misourri at columbia presented its at
evaluation protocol at c-sun. the conference paper was distributed on
this list in January. Please contact them for their evaluation form. I
