I believe the vast majority of these cases were amateurs who tried making
it at home, and didn't quite know what they were doing, or simply used the
wrong kind of fish.  Japanese restaurants have trained chefs, and select
the fish carefully.  One chef/owner I know has fish flown in fresh from the
Far East every morning; he goes out to the airport at 6AM to pick it up.
