This won't help too many of you - but El Al - the Israeli airline is a
good bet. First of all, all the food is kosher anyhow - so that's the
first hidden milk hurdle out of the way. They have a range of about 18
different meals you can specify - one of which is dairy-free. They also
do stuff like bland diet, low-fat, fat-free, and a ton of other options
I would never have thought to ask about. They do a kids meal which is
hamburger and chips - by definition dairy-free on El Al.

Last time we flew, I asked to speak to the food technologist in charge
of the kitchen and food preparation at El Al. They were very helpful and
answered all my questions about what could possibly be in each meal I
might possibly get (meals change every flight so they couldn't tell me
exactly what would be on my flight). Worth the effort.
