I ask:

   Does it contain any dairy/milk products?
   Does it contain any products derived from milk?
   Has it been made using equipment that has been used to make anything
   containing milk products or milk-derived products.
   What other products do you sell that are also completely dairy-free?

If there are some items that are on a list of potentially milk-derived
ingredients, e.g., caramel coloring, I ask specifically if they ever use
one that has been derived from milk.

For us, we also ask the same questions about nuts and eggs.
