The difference is that on a ketogenic diet, you simply avoid sugars and
starches so as to force your body to use fat as the primary fuel rather
than sugar.  On a paleo diet, you are attempting to mimc the diet of our
hunter/gatherer ancestors as much as possible in today's world (not easy!).
 So, before you put something in your mouth, the question isn't so much
"does this have carbs?" as (to paraphrase from Neanderthin) "if I were
naked in the wilderness with a rock or a sharp stick would I be eating
this?".  So, obviously, things like dairy products and refined foods which
may be allowable on a ketogenic diet, are not allowed on a paleo diet.

Paleo can be done as a ketogenic diet if you are trying to lose weight, and
indeed, is undoubtedly the most healthy method of doing so.  Of course,
with the food selection available these days, it might be more correctly
described as the least unhealthy method.
