Jill,
I only did this for a short time, but I just used a small notebook
that was easy to carry. I just used 1 page per day and kept track
chronologically of consumption, symptoms, naps, etc...
i.e.
7 am woke up, nursed
8 am breakfast: 3oz kix
4oz diluted OJ
1 slice whole wheat toast
9am: cranky for no apparent reason
10 - 12: nap
12 noon lunch:
etc....
