I'm a Canadian Celiac and fly fearly frequently and here is my experience:
 
Canadian Airlines International will provide gluten free meals (if ordered
in advance).
 
They are very accommodating in trying to help and their meals are
excellent. I have never had any problems after. They will make a note in
their data base (i did through a travel agency) that you require gluten
free food on your flights and even seat preferences.
Canada 3000, a charter airline, will also provide gluten free food (if
ordered in advance).
 
I have found their food realiable as well and very well prepared.
KLM provided me with gluten free food but there was a dinner roll packed
with that looked regular bread. I asked to have it taken back and they
substituted it with onboard salads. I would not rely on their "ghluten
free" meals.
