I was asked about these cards and I got them from my local chapter but they
can be ordered, I forget how much they cost, from the Canadian Celiac
Association by calling 1-800-363-7296.

It is a fold-over business card size info card.

I find it very useful to hand to the waiter and then elaborate on it with
questioning the menu items, or ask them to recommend something and then I
ask my detailed questions.  It really helps by giving them the card to take
to the cook for recommendations prior to being seated, that way not so much
time is taken up to order.
