I can't address your first question nor do I have a definitive answer
for the second one.  I can say, however, that I drank a mug of hard
apple cider at our local renaissance faire several weeks ago and had
no problems.  After I drank it, the thought occurred to me that 'hard
apple cider' could be just what its name suggests but could also be
one of those 'malt beverages' with apple cider flavor added.  Before
I was aware that commercial wine coolers were actually malt beverages
in disquise, I drank one (a red sangria, I remember) and had quite
a reaction.
