Karen - I seem to have good and bad periods on the diet...sometimes
I feel great, then I slip up and it's like going back to square one.
I noticed a dramatic improvement recently when I stopped eating foods
with annoto, but then something else set me back and I'm not even sure
what.

It sounds to me like you might possibly not be following the diet
well enough--those can be celiac symptoms. Or you might be sensitive
to other things, have refractory sprue (which I believe is very
uncommon--it's sprue that doesn't repond to the diet) or have some
other problem entirely. You might want to check with your doctor about
getting another biopsy, or about some of the other tests that might
show if there's still gluten in your diet. Are you following the
Canadian diet? The American diet is apparently much stricter, you
might want to give it a try.
