Harry David is right.  Vo sounds different also you will get beeps sometimes but not others.  The beeps are sent by the cellular network when a call drops.  It might help if your friend wore a ear peace.  I have a good single ear one I use if you or others want the link let me know.

Hi all,

Well folks, tried to hang up a call on the iPhone, and the caller hung up, and there were no beeps at all to indicate the call was ended.

We then turned on voiceover, and no beeps, to indicate the call hung up.

We had absolutely no idea what was going on.

So, I called Apple and said, "we need a sound to indicate a call is hung up, or, to indicate the call was dropped.

They had absolutely, no idea how to do such a thing, and told me, "in the settings, there is no sound to indicate those things happened.

I want to put a friend into an iPhone, and if we cannot get this to work, then it's not going to happen.

I thought about doing a shortcut keystroke, like space bar, just 1 key to hang up the phone.

But I've never made a shortcut before.

My sighted friend knows when a call is ended or dropped, because it says it on his screen.
