I would go with something like this:


Helping people with disabilities rediscover their lives.


or


helping people who are new to disability get back into living.


Like others, I don't like "the disabled," and I'm a little hinky about 
the idea of making life normal again, but I also know that people who 
aren't used to living with a disability do think in terms of normal and 
not-normal, so it's useful to acknowledge their thinking in ways that 
are more palatable to those of us who are used to living with a disability.
