Hello,

I was able to take the survey with NVDA and Firefox's latest version without a problem. Pressing space bar on the choice resulted in the item being checked. It was a standard radio button with nothing fancy that could hinder screen readers.

I tried to take the survey but when I tried to check my answer for 
question 1 it would say unchecked no matter if I pressed space bar or 
enter.  somone needs to learn how to make a survey.  I would highly 
reccommend nvda a 10  because it's so flexable and there lots of add ons 
and 3 version s per year.
