Have been a 20 plus year user of Kurzweil and would not use anything else.

Personally I really like Open Book. It is easy to use, accurate, flexible,
and does some nice things like help one find on-line books, and photocopy.
If you use JAWS it interfaces seemlessly. If you use another screen reader,
then you do have to put that screen reader to sleep when using Open Book.
