I was able to play it with IE8 and Jaws 10 with Windows XP.  I was not
able, however, to tell whether my selection was checked on that Google
form.

It is beginning to appear as though this, as in many other recent
instances, accessibility will require us all to run many alternatives
for web browsers and many alternatives for screen readers. A new(ish)
trend.
