Interestingly, I logged into my amazon account on a brand new computer, so I
didn't have any cookies at all, and had no request for it at all.

I wonder if it's just a random issue?

one is most likely to get one if either you don't save cookies or if 
signing out.
I got this once -- a couple of weeks back -- and was able to fix things by 
importing a backed-up cookies file.
So, my suggestion is to periodically back up your browser's cookies, 
remain signed in to Amazon and avoid intentionally signing out.
