LookTel Recogniser does not OCR signs or describe images.

LookTel Organiser has the user create a database of images. The creators recommend a sighted person does this because a sighted person can ensure only the item is fully in the picture. After the database is created, when the app is running, aiming the iDevice's camera at the item will trigger a match in the database and the name of the item will be announced. The same basic technology is used in LookTel Money Reader, but in Recogniser, one can add any image to the database.
