Thanks, Dan and Don, for responding.

The way I got to the Sign-in page with the CAPTCHA, which I will try to 
paste into this e-mail, was to search for an item, add it to my cart, 
and then Proceed to Checkout.  It asks for your username and password, 
you click Sign In, and then I got this (my apologies if this doesn't 
come through well via e-mail):
