Eddie,

Speech should not go away when you run a system update on Android. I 
seem to remember that someone on the Eyes-Free list lost speech, but he 
was using a different TTS, which needed to be updated. If you're using 
the default Google TTS, you should be fine.

Remember to plug your phone into a power source before updating. The 
update won't run if your battery is around 50% or less.

You should be able to dismiss the system update notification by tapping 
Remind me Later. If that doesn't work, try double-tapping Back or Home.
