I'm not an Outlook user, so I don't have any wisdom on adding attachments 
aside from trying the Insert menu.

My post is on getting into the Quick Access toolbar. If you're a Jaws user 
(and I'm assuming this works for everyone else), You can press alt to get to 
the menu bar, then shift+tab a couple of times to get into the Quick Access 
taskbar. For some reason, hitting tab doesn't do the trick, but hitting 
shift+tab works just fine.
