I always have problems with JAWS and EQSL and gave up and use LOTW.

Hi
     Well, After two days I decided to fill in my contact to Hawaii on
eqsl.cc. Things went relatively smoothly except for the three boxes I have
to type information in to. I am running the latest version of JAWS and
windows 7 but these boxes appear to have no labels. Does anyone know which
information they want in each box?
