I wouldn't respond to this as it sounds fishy and have never received
anything like this either.
If I thought this necessary to do, here is how I would play this. Fill
out the form using a bogus password. If it accepts it, then you know your
suspicions were on target.
If it doesn't accept it, then you need to do further checking. A
legitimate verification request should allow you to do so from within your
gmail account without needing to use this supplied url.
