I have used Survey Monkey in basic mode and it was successfully used by
screen reader users. I found the drop down boxes that might have been
easier for switch access caused really difficulties when collating results
if they were used in a matrix.

Best wishes E.A.

Subject: Accessibility of Survey Monkey (and other on-line forms)

How accessible are surveys generated via Survey Monkey and other on-line
survey services?

Thanks for your thoughts,

Alan
