I feel compelled to clarify the poster's summary for 'Canada'. 
I sent the poster entries from the archives and the search link I used. 
I mentioned I live in Canada so I do not have the problem (although 
when I lived in Arizona, there were some that were denied).   

The denial of coverage does not apply to Canada. 

Canada has universal medical coverage and NO ONE is denied basic 
medical insurance because of a diagnosis.
