Hi All,

Yes, American Thermaform is still alive and well.  They are located in 
California and their phone number is:

     800-331-3676.

I use three-hole punched thermaform all the time for various things and I 
believe they have fan-fold for braille printers as well.  I purchase it 
prepunched from them in boxes of 1500 sheets I think it is.  Anyway, it's a 
better quality than I remember the older stuff being and does not make your 
fingers tired when reading like the older stuff used to do.

I find that it works well either in a brailler or with a slate.  I use the 
brailler most of the time, however.
