Here are the responses I received.  Thanks all!

The Slanted Door in the Ferry Building is the most knowledgeable restaurant 
I've ever come across in regards to gluten. Every time I go, the servers are 
very helpful and I feel completely safe from cross-contamination. 

I had a gluten free meal at E & O Trading Company at 314 Sutter Street. They 
actually have a wheat free menu that I adapted to GF. It was excellent.

I was there a bit over a year ago and got an excellent meal at Millenium 
<http://www.millenniumrestaurant.com/>. Great upscale vegetarian, without 
any attitude about their food. In the same general neighborhood is a Westfield 
shopping mall. Choices there are Out the Door

>, good SE Asian fusion for a reasonable price, and Cocoa Bella

29139.html>, with a wide variety of artisanal (expensive!) chocolates, 
scrumptious hot chocolate, etc. I'm sure there are others, but these are the 
ones that I still remember from that trip. All three were knowledgeable about 
their ingredients and willing to accommodate.

Most nice restaurants in SF can accommodate the diet. Cafe Gratitude is GF 
except the beer, Chipolte- everything is GF except the tortilla (we get bowls), 
Chevy's has a GF menu- just have to ask, I've eaten @ the Slanted Door, 
Boulevard, Hayes St. Grill, Ruth Chris Steak House all without a problem. Many 
Bay Area restaurants are now becoming used to this request.

There is a large support group there that you can find by googling "celiac 
support groups California"

If you've got kids, I found if I called Rainforest Cafe near Fisherman's Wharf, 
they are so darned accommodating that they offered (if I let them know what 
day and time I'd be there) to go out and buy a potato to make me a plain 
baked potato, and they made my steak in a separate oven/grill, away from all 
other food. I wrote to corporate ahead of time (and afterwards), and the chef 
from Rainforest called me...and had my email printed out. 

I also have eaten at the fish restaurants on the wharf where Alioto's is and a 
one upstairs on Pier 39????--the one with the shops. When I went out there 
for conventions (annually, for years), I had some success at Lori's Diners for 
breakfast...really depends on the staff. If you find a good Lori's, stick with it. 
They're all over the place, even at the airports and malls.

I go to Max's Deli's...all I get is a plate of corned beef and pastrami and their 
trio of potato salad, cole slaw and forgot what else...cucumber salad? 

I've been told that you can get a carb-free burger, wrapped in lettuce at In 
and Out Burgers, but the occasion hasn't occurred...in all the years we've 
gone out there.
