Wishes for best of 2009 for everyone!
 
Several people asked for a typical letter they could draft to give their
doctors to re-type and sign and keep on file to send with IRS documents and
special travel needs.
 
Here goes:
 
Your doctor will retype this on his letterhead and his original signature.

To Whom It May Concern:
 
Miss/Mrs/Mr. (full name), of city, state, has been my patient since blank.
He/She suffers from Celiac Disease (or gluten intolerance, or gluten
sensitivity, whatever your doctor wishes to classify it if you have not been
diagnosed by biopsy), and must comply therapeutically with a strict
gluten-free, casein-free, whatever else you cannot consume free diet.
Special emphasis should also be placed on organic plant and whole foods.
Note: If you have other very serious medical conditions you should list them
here as well if Dr. is a GP, or say, for heart, same such letter from your
cardiologist would be appropriate further noting you require a low fat,
heart-healthy diet as well.  If you require certain medications like
thyroid, or cancer drugs, pain or anxiety medication (lomotil is a
controlled substance as well as xanax) diabetic meds, this can also be noted
as concisely as possible in the letter, or you can have your doctor draft
such a separate or inclusive letter, say for travel purposes. If you have
other disabilities (severe allergies to be included) include them here as
well, or get additional letter from specialist.  
 
If you have any questions about my patient's medical condition please call
my office at the above letterhead.

keep original letter in clear plastic folder in safe place - you might ask
your doctor to print you out two with original signature if you have to
carry originals while travelling - copies of this letter can be sent with
tax form used to deduct difference of gluten-free product from what
non-gluten cost would be less non-celiacs in household, difference between
organic and regular fruits and veges, some whole foods, in cancer case the
entire amount for top 10 cancer preventing foods, etc.  You must scrutinize
your grocery bills but I think it's fun - I like saving a buck, especially
in these times.
 
This letter can also be used for special travel arrangements.  Hope this
