HI Steve,
I think the product is made by Niles Audio.

The one I have has two rca phono jacks as outputs. I run these into the 
tuner input of my Icom preamp.

It also has six inputs also fed by rca phono jacks.

It comes in a solid metal case and has a rugged rotary control in the center 
of the front panel for selecting input sources.

I got mine for around $100 plus tax from a local California audio dealer.

Try doing a google search for Niles Audio and see what comes up.

Also, many high end hi-fi dealers carry Niles products.

I think they are used in home theatre applications.
