If you have Kurzweil Version 11, there is a mode which might help in filling out 
forms like this.

I have a 4 page printed insurance form which I would like to fill out on my
computer rather than having someone fill it out.  I have Microsoft Word and
Word Perfect.  Word Perfect has a scan function under Acquire Image which I
tried but form came out a mess.  OmniPage didn't do it either.  Please
respond off group if you have a solution.
