I know we're talking about a child, who is probably just scared of needles,
but I feel getting a refill doesn't hurt, doesn't the medicine get injected
through a tube to the pump.

Botox does not make you a zombie. You are thinking of something else.
Botox is a miracle and only goes to the muscles. The shots hurt and
we would not let them do them to Case again without sedation. Doctors
differ on this but use a light sedation and consider trying it. It
wears off so you can then be informed about whether or not you would
want to continue.
