Hello:
This is a survey about woman’s healthcare experiences among women who have
cerebral palsy. This is a nonscientific survey. No names will be attached to
the results. The results may be used in research and is being carried out by
the project on chronic illness and disabilities (CHAD) at Idaho State
University department of Anthropology. All information will be kept confidential.

This survey is for women of 18 years or older.
Please describe as much as possible.
Has you doctor preformed regular cancer screenings (i.e.: mammograms,
pap-smears, colonoscopy) for you as appropriate for your place in life?
Did you have to use any special techniques such as sedation, or sending you
to a high-risk clinic to have screenings preformed?
If you have never had these screenings preformed what is your feeling about
this issue?
What has been your experience with your doctor when it comes to issues
concerning women’s healthcare?
Are you still looking for a doctor to address this topic?
