I believe that I still have it.  Pat sent it to me a few years 
ago.  I just have to find it.  Perhaps Pat can do it faster, since 
she is the one that gave it to me in the first place, but I'll give a 
look.  A friend of hers had typed it up and Pat sent it to me as an 
attached file.
