Snakebite
Scorpion sting
Malaria & other insect transmitted diseases
Parasites
Infected wounds
Complications from pregnancy
Murder
Warfare
Getting eaten by tigers / crocodiles
Getting gored or trampled by elephant (or equiv) while hunting
Friendly fire while hunting

Hmm, ok, that's all I can come up with right now, but that's a 
pretty good list.  Off the cuff I'd say as a paleo human you'd
have at least a 50% chance of dying of one of these before
you reach the limit of your old age, and that's after you've
survived childhood.
