It would appear that K.Y.W. does want you to fill out the form before they
will permit you to listen to the stream.  I had the same problem even when I
tried to access it through Bill Sparks's website.  I filled out the form,
but I really don't like that at all.  It should be up to the person as to
whether or not they want to fill it out.  I realize that it is primarily for
statistical use, but it still should be an option.  73.
