Get the bracelet version and mount it on a bracelet with a sufficiently
difficult to open clasp such that the child will either not understand how
to open it, or give up before succeeding.  I used to wear a Medic-alert
necklace, and once got it caught in the trim of a capsized canoe while under
water.  I quickly reached up and broke the necklace so I could surface :)
