Hi Matthew,
Here are the instructions from the echo link site.
These instructions also work on the di624 and di 524 routers. but the
instructions did have the tcp and udp ports listed backwards, they are
correct in this list.

D-Link DI-604
1. Using a Web browser, bring up your router's configuration page. By
default, the URL is 192.168.0.1, the user name is "admin", and the password
is blank.
2. Click the Advanced tab.
3. Click the Applications button.
4. Click the Enable button.
5. After Name, enter "EchoLink".
6. After Trigger Port, enter 5200.
7. After Trigger Type, choose TCP.
8. After Public Port, enter 5198-5199.
9. After Public Type, choose UDP.
10. Click "Apply".
