After you re-load the operating system, and before you connect to the
internet, do you install a software firewall or do you have a hardware
firewall/router/switch?

It's possible that if you are connecting with an unpatched windows system
you are being infected each time you hit the internet.

We tried an experiment here at work, connected an unpatched pc to the
internet with no firewall and it was infected in less than 1 minute.
