Try this:
Put your cursor at the beginning of the first heading that you 
want included in your table of contents, and press 
alt-shft-left-arrow. JAWS should say, "heading 1". This applies 
the Microsoft formatting to that text, which is Bold. Press 
alt-i, go to Reference, then Index and Tables; and make sure 
you're at the Table of Contents tab. Make sure that Show Page 
Numbers and Right-Align Page Numbers are checked. Now follow 
these same steps for each of the other headings you want included 
in your table of contents.
