Another tool I'd like to suggest is Lead Tools Eprint available at:

It seems that while most pdf documents are moderately readable with
screen readers, if the if the security options are selected,  screen
readers are unable to do anything with the document.
Leadtools E Print sets itself as a PDF virtual printer. When in Adobe
Reader, choose print from the file menu and point the printing to the
Eprint Printer.  Press ok and then the Eprint program will ask you what
document format you would prefer.  Choose Microsoft Word as Eprint will
put background imaging as a nag for you to buy the program.  The screen
readers ignore the images, so that's okay. If you choose text format,
the nag is put right into the text and is quite annoying.
It's a very useful tool.  I've been able to access many PDF files that
were inaccessible to me via the normal Adobe interface.
